GP LETTER OR CERTIFICATE FOR SCHOOL COLLEGE UNIVERSITY FOR ABSENCE EXAMS COURSE WORK OR PERFORMANCE
Thank you for your query regarding the above.
NHS GPs have no professional involvement in education or school attendance monitoring, and do not normally provide letters or certificates confirming absence or impaired performance, as the parent, guardian or carer's explanation will usually allow the education centre to ensure that any health-related concerns are addressed.
Please note, requests for sick notes (MED3s or fit notes) in this situation are inappropriate, as they are legal documents for employment and statutory sick pay purposes only and therefore inappropriate for students.
If a GP letter is still required by the parent and/or education centre, please note that this constitutes private work, and as such will incur a fee, payable in advance. GP letters are restricted to factual information only and no opinions will be provided. Should such a letter be required, please contact the surgery, providing a copy of the written consent of the parent, legal guardian or carer if the student is under 16 or the student's own written consent if they are over 16.
Regarding exams, including missing exams or impaired performance due to medical reasons, please direct your education centre to JCQ , and in particular, form 10 for special considerations and form 14 for self-certification following missed exams. GPs are not required to be involved in the completion of either form.

Yours sincerely
